Update:After some experiments, I figured out how exactly the bug works. A unit only gets the flamethrower's burnout cover bonus after it moves into the smoke. Once it has moved, the green bonus marker appears. Afterwards, you can TTC the unit in and out of the smoke tiles and the marker will disappe...

When I use the flamethrower, I see smoke but get no green cover bonus marker on soldiers inside of it. Just to expand on this point further: the wrong tactic is to set your guys on overwatch, then have one of your guys shoot or lob a grenade to open and hope your overwatches kill them all. The right tactic is to watch their patrols, then move your guys into the patrol path and then overwatch in it. ...

My assumption is: Defense conteracts Aim, so for every point of Defense, the to-hit-chance is lowered by 1%. When the aim bonuses are high enough, defense is completely neutralized.
