Tons of mods.
If a mission pops with less time left than the flight time to reach it, the mission will disappear while in flight, and the squad will be stuck infiltrating permanently.
For example finding a mission with 0 days 1 hours left.
Should be fairly trivial to reproduce, don't even have to find the mission late, just carefully pass the time on a found mission, and park the Avenger at a long flight distance.
